EARLY ARRIVAL - INTAKE FORMS
If you are a new client, please arrive a few minutes before your scheduled appointment, or complete the intake forms found at https://www.tealcenter.com/about/intake-forms.
If you have booked an appointment for Acupuncture, Arvigo Mayan Abdominal Therapy, or Craniosacral Therapy, please download our forms and complete prior to coming to your appointment as these forms require more than a few minutes to complete.
24 HR CANCELLATION/RESCHEDULING POLICY
Appointments missed, cancelled or rescheduled with less than 24 hours notice will be charged in full. To avoid being charged for a missed appointment, we invite you to send a friend or family member in your place. Also, if we can fill appointments that are missed, cancelled or rescheduled with less than 24 hours notice the client will not be charged. It is your responsibility to remember your appointments.
APPOINTMENTS GREATER OR LESS THAN 60 MINUTES
Requests for these appointments will be sent to our staff for approval. Since the default time slot is 60 minutes, we will have to see if we can adjust the times. This is not yet an appointment until confirmed. Please check for follow up emails that may require your action.
If the appointment request is accepted, there will be a change in appointment start time or end time. We will send an email and will hold that spot for you for 24 hours. Please respond within 24 hours to accept or decline the appointment.
In some cases, the therapist may be able to accommodate your requested appointment, but only for 60 minutes. Please respond within 24 hours to accept or decline the appointment.
If I have not accepted or declined the appointment 24 hours the appointment will become available again to the public.

For Randolph Towers
The Teal Center does not validate parking. Parking options include metered parking, which is available on all the streets around the building, or some limited complimentary parking, which is available in the parking lot behind the building.
In order to gain access to the parking lot behind the building, you must first pull up to the circular roundabout in front of the building.In the lobby area, inform the concierge staff that you have an appointment at The Teal Center. They will provide you with a parking pass and a token. You will then have to pull out of the circular roundabout, take a quick left on 9th St and another left onto Quincy St. The entrance to the parking lot is on the Quincy St. side of the building. The token will open the gate to the parking lot. Place the pass on your dashboard. Please be aware that parking is first come, first served and is not guaranteed. From the parking lot use the gated entrance that will lead you to the lobby. Once you’re in the lobby, take a left, you will see a set of elevators. The Teal Center is located on the 2nd floor in Suite #230.
Virginia Hospital Center

There are two parking options: Either metered parking, which is available in front of the hospital, or the hospital garage parking, which costs $5. The Teal Center does not validate parking.
To park your car in the hospital parking garage (two locations):
- Park in the Zone C Lot, 3rd level, walk through medical office building, across the skywalk, down a short stairway into the hospital. Take the first right (exit sign overhead). You will see The Teal Center straight ahead of you. You should also start seeing signs for us once you cross the skywalk.
- Park in the Zone C Lot, Ground level, walk across to the Outpatient Oncology entrance. Follow the corridor straight ahead of you. It will turn a couple of times and take you to the elevators. Go up one floor to the Lobby level. You will find us just steps away, around the corner from the elevators to the right. You should see our glass doors and a small table out front as a landmark.
